Admission process

If Resident Requires Oxygen

Lisaard House has an agreement with VitalAire to supply oxygen needs and services for the residents at Lisaard House. If the resident is presently receiving home oxygen from another provider, VitalAire can help transition the resident from their current supplier.

Medications

All prescriptions and supplies are provided by Preston Medical Centre, the present CCAC provider in the Cambridge area. If a patient is coming from a hospital setting, the prescriptions will need to be faxed to Preston Medical at 519-653-9232 prior to the patient's arrival. Lisaard House does not keep stock medications; delivery from Preston Medical is usually late afternoon or early evening. Residents are encouraged to bring their current medications from home.

Transportation

Lisaard House prefers that residents be transferred early in the day. If a resident is coming by “About Town” from home, families will need to make such arrangements. Arrangements should also be made for someone to bring a resident's personal belongings from home.

CCAC

Lisaard House assumes that all residents coming to the hospice will be on CCAC services prior to admission to the Hospice.

Other Considerations:

  • Lisaard House is not on a bus route and it is a considerable walk from the closest bus stop; as such, families who do not drive will need to make appropriate arrangements for transportation to and from the house.
  • Because Lisaard House is close to a highway and woods, a person who has wandering behavior may not be appropriate for a Lisaard application.
  • Because of the location of Lisaard House and the fact that there are only 2 staff members on at night, a patient with extremely aggressive behavior may not be appropriate.
