Admission
criteria
Who
can be a resident at Lisaard house?
Someone
who:
- Is
an adult dying of cancer who is no longer receiving curative treatment
- Is
unable to live alone, or has inadequate support at home, or has
caregivers who are overextended
- Is
willing to be assessed for eligibility by Lisaard House staff
- Has
a power of attorney for personal care
- Has
indicated he/she wishes symptom management for comfort only.
- Has
a primary care physician who is willing to transfer care to a Lisaard
House palliative physician
- Is
on CCAC services
- Has
care needs that can be met by the services available at Lisaard
House (these may include measures for pain & symptom management
such as oxygen, intravenous fluids, gastric tubes, etc., but will
not include other, more aggressive, treatments).
Upon
reassessment and consultation between the resident’s family, CCAC,
& Lisaard, a resident whose health status improves or stabilizes may
be requested to return home/or transfer to another residence.
Who
is not an appropriate resident for Lisaard House?
Someone
who:
- Does
not meet the above criteria
- Lives
outside the Waterloo Region – unless significant family members
live in Waterloo Region.

Has behaviors that are difficult to manage, such as aggressive behavior,
violent behavior, wandering behavior, and serious confusion
- Has
requirements that cannot be met by the services available at the
house in cooperation with the services of CCAC
- Desires
active, continued treatment of cancer
It
is the desire of Lisaard House to give consideration to each individual
request.
